Clinical software is judged on traceability before features. A recommendation that cannot be traced back to the document it came from is not usable evidence, and retention rules mean data outlives the system that produced it. The technical consequence is that provenance has to be recorded at write time, because it cannot be reconstructed later.
What we have built here
We built acquisition over public product sources where every record carries where it came from and when it was taken. The lesson was that provenance is a schema decision, not a logging one: added afterwards it is a guess, and a guess is what an auditor will find.
